SHORT DESCRIPTION:  2012 NERC Cause Analysis Training

INTENDED AUDIENCE:  ERCOT Market Participants

DAY AFFECTED:  October 30, 2012, 08:00 - 15:00

LONG DESCRIPTION:  ERCOT will host a NERC Cause Analysis Training class on Tuesday, October 30, 2012, from 08:00 to 17:00 at the ERCOT Taylor Control Center. This course is recommended for those involved in NERC Event Reporting and Analysis. A consistent analysis approach will facilitate the accurate trending of events and improve timely, effective resolutions. The agenda for the class is as follows:

Introduction to Cause Analysis

Five phases of Cause Analysis:

   I.   Data Collection
   II.  Assessment

   1.    Cause Analysis methods covered

   a.    Task Analysis

   b.    Change Analysis

   c.    Barrier Analysis

   d.    Events and Causal Factor Analysis

   e.    MORT/Mini-MORT

   f.     Kepner-Tregoe(r) (K-T)

   g.    Fault Tree Analysis (FTA)

   h.    Techniques for Human Error Rate Prediction (THERP)

   i.      Cause and Effect Charting

   j.      Human Performance Evaluations (HPE)

      2.        Cause Coding

   II.  Corrective Actions

      1.        What constitutes appropriate corrective actions

   III. Inform

      1.        Reporting of events/occurrences on the Bulk Power System (BPS)

   IV.  Follow-up
